An eyelid lift, known to plastic surgeons as “blepharoplasty”, removes excess skin and fat from the upper and lower eyelids. The surgery is tailored to the needs of each patient, whether the problem is a little extra upper eyelid skin, or serious “baggage” on the lower lids.

The Upper Eyelid Lift
Dr. Cassileth performs this procedure using an incision hidden within the natural fold of the upper eyelid. The perfect upper lid has a clean contour, with a crisp and visible crease, no hanging skin or fat, and no tightness to the final appearance of the eyes. Dr. Cassileth strategically places the incision to follow the natural fold of the upper eyelid, creating an inconspicuous scar that is practically invisible. The excess fat and hanging skin are accessed through this incision. The final result should look natural, youthful, and never “done.”
Some patients may have such severe hooding of their upper eyelid that it obscures their vision. Diagnosis of impaired vision can be quickly determined by an opthamologist performing a visual field test. Some insurance plans provide coverage for eyelid lift to correct impaired vision. The Lower Eyelid Lift

To get rid of extra, wrinkly skin under the eyes, Dr. Cassileth removes the unwanted skin through a tiny incision hidden just beneath the lower lashes.
Puffy eyelid with excess fat
If you have puffy under-eye “bags,” Dr. Cassileth’s precise technique redistributes the fat under your eyes, creating a smooth, youthful contour and giving you a well-rested, revitalized look. This surgery permanently fills the deep valley under the eye with fat from your puffy eye bag, creating a smooth transition from the lower lid to the cheek and putting the fat back where nature really should have put it in the first place. Because the incision is placed just inside the lower eyelid, there will be no scars on your skin. Performing the surgery using this internal (or “transconjunctival”) incision is superior to an incision straight through the lower lid because it avoids problems with lower lid position or eye dryness after the surgery.
Preparing for your eyelid lift
Dr. Cassileth will perform your eyelid lift in our on-site surgical suite, usually using general anesthesia. Depending on the procedure and your comfort level, Dr. Cassileth may be able to perform the surgery using a local anesthetic (similar to when the dentist numbs part of your mouth). This method is only used if it is a reasonable and safe option for you. Patients are able to return home the day the procedure is performed.
What to expect after your eyelid lift
You will have a single suture visible after your surgery, but your eyes don't need to be covered. You will have some light swelling and may have bruising. To speed healing and relieve discomfort, we recommend that you use cold compresses and elevate your head when lying down. You will need to clean the eye area carefully and apply moisturizing eye drops for 10 days after surgery. We will provide a list of activities and environments that you should avoid during the first weeks after your surgery, but generally, all exercise should be avoided for the first week. Any non-absorbable sutures are removed five days after surgery.
